PCAP (Primary Care Asthma & COPD Program)

The Primary Care Asthma & COPD Program (PCAP) is an evidence-based education and management program. The program has proven positive outcomes and all tools and resources are aligned with the latest Canadian Thoracic Society (CTS) guidelines. The program model has been adopted across Ontario extending its reach to over 180 primary care sites. The Lung Health Foundation offers implementation support and enhancement of your respiratory health program.

Emergency Department Asthma Care Pathway

The Emergency Department Asthma Care Pathway (EDACP) is a standardized approach to asthma care delivery in the emergency department that is based on current clinical guidelines. The Pediatric EDACP supports children ages 1-17 years with a history of asthma or previous episode of wheezing requiring treatment. 

The Adult EDACP supports those 16 years and older with a history of asthma, chronic obstruction pulmonary disease (COPD) with asthma or a previous episode of wheezing requiring treatment. The pathways ensure, appropriate assessment of the severity of the exacerbation; evidence-based asthma treatment, patient education prior to discharge, comprehensive discharge instructions and clear plans for follow-up care.
