Primary Care Asthma and COPD Program
The Primary Care Asthma and COPD Program (PCAP) is an evidence-based education and management program. The program has proven positive outcomes and all tools and resources are aligned with the latest Canadian Thoracic Society (CTS) guidelines. The program model has been adopted across Ontario extending its reach to over 180 primary care sites. The Lung Health Foundation offers implementation support and enhancement of your respiratory health program.
